Yes it is very real. A result of diluted sodium concentrations in the blood. Most often seen in a weight gain in endurance exercise lasting over 5 to 7 hours. Symptoms are frequently misdiagnosed as severe hydration, thereby, exacerbating correct medical treatment. Since I run "ultra" distances and hang around people that do the same, I'm very familiar with this condition. When performing long endurance exercise, I use sodium capsules (electrolytes, if you will--Gatorade doesn't provide enough sodium at correct concentration levels) to help the stomach process fluids in the stomach. It turned out I had hypercalcemia due to a tumor and that I craved water because it flushed out the calcium. The surgeons told me the excess water probably saved me from severe organ damage due to the calcium, it certainly did no harm. I've dealth with psychiatric patients with hyponatremia. We were totally exposed to the sun all morning. (We camped midway.) The temperature was in the '90's. When we finally arrived at the water source, I drank down about a gallon of water, waited a little bit and drank more. The only shade was from a large boulder. We sat there for a couple of hours. I drank until I was sick. It's not fun. At the time I was just aware that I was really really thirsty. Growing up in the desert I learned not to do that - not to drink water that is really cold or too much at one time. But I ignored what I knew because I was so thirsty.
