“I'm a college student studying psychology and then I'm going into a 2 year nursing program to become a psychiatric nurse. This is my last year to complete my bachelors. I also work as a mental health case manager at Penn Foundation in PA.

As far as dually diagnosed.. I assume you are talking about a mental health dx and a substance abuse dx?

We have an entire Recovery Center that is based for the DD. We also have 2 DD houses (male and female) that house 8 individuals in each. It's called the Village of Hope. The treatment plan is that they can live at the house (it's like a mansion... absolutely gorgeous) for a period of up to 2 years (and in some cases longer) as long as they remain in treatment and work on their MH issues and the SA issues by going to meetings, partial, outpatient, etc: whatever is needed on a case by case basis.

We also have a Mobile Engagement Service (MES) that we run out of the Recovery Center for DD clients. They are basically therapists that do outreach with the patients themselves and also work with a network of people in the MH and SA fields and even the patient's family and friends. They provide a support web for the patient and also help with case management issues as well as their MH and SA problems.

I'd be more than happy to give you the numbers or the web site of the facility that I work at... I'm just merely a case manager at the moment, so I don't really know all the options that we have readily available to the public yet. But the programs that I do know about have a pretty high success rate.

“Thanks for the info ,scorchfire the hospital I work at has pact teams that do community work,unfortunately their not that succesful,poor funding,under staffed.and they often work under a diffrent ministry.(ministry of health,where as the homes are Minstry of community and social services ,they don`t get a long)
The floor I work on is for the mentally retarded who also have a mental illness.They come to us due to behaviour problems in theircommunity settings.The ward is presently becoming nothing more then a short term warehouse.We are becoming very limited in what we are now allowed to do program wise,not only that but many of the Community homes are not allowed to,implement our programs.We just had a client come in who frequently trashed his apartment and all the staff wereallowed to do was watch and clean up after him,the best they could do was have him stand in a corner for 5 minutes.That doesn`t helphim or the staff and costs the taxpayer $$$$$.He ended up severrly hurting several staff,before coming to us.We stopped implementing a program when the group home informed us that,they had np plan of action,thingd were to remain the same!!! how do you help them or him ???.He was dicharged a couple of days ago,when he got back to his place ,he promptly trashed it completely.It`s geting frustarating.The new approach is to treat them with kiddie gloves on.It`s not working.....We`re supposed to help these people to get back to a level where they can function in the community without further endangering the lives of those around them or endangering them selves.How do you treat aggressive behaviour thats been allowed to go unchecked for years,by saying now be a good boy ,thats not nice to punch someone....thats what we are geting to.Staff like me are deemed old fashioned and rigid,cold and unflexible. Ugh...sorry scorch fire I`m ranting..I apologize,just a little frustarated thats all. I better stop here or I`ll still be typing (two finger) 6 hours from now. Best of luck on your exams.”

I don't have much experience with MR patients, but we DO have MR case managers and staff. What about placing them in residential homes that are exactly for behavior like they are expressing?

My biggest accomplishment was pulling a script for an hour long dramatic series out of my a s s in 24 hours. Anyone that knows anything about scriptwriting will tell you that this is impressive. Even got a A.”

“Scorch fire...we have residental homes,unfortunately they sre mostly run by the Minstry of Community and social services,hence the problem.They frequently will allow no behaviour programing and work very short staffed(generally one staff).Our whole system needs a major overhaul.The Ontario Goverment is dumping Mental Health and forcing the private sector to take mental health beds.We were taken over by a catholic hospital 2yrs ago,it`s been down hill since.They didn`t want us then and still don`t.”
