According to Michael Quinion of http://www.worldwidewords.org/weirdwords/ww-tit2.htm, TITCH is mainly a British and Australian colloquial term.

The original term of 'titch' referred to a person by the name of Harry Relph. Relph was a famous music hall performer, born in 1867. He apparently only grew to a height of 4 ft 6 ins tall. His childhood nickname was Tichborne or Little Tich and, as he became more famous on stage as an actor, other little people started to be called titch.

The word is also used to refer to a 'ballboy' in sports.

"Mad" is used as a modifier to to your pull to denote that your level of pull has been heightened to a very high level as to appear insanely high.

To elevate your pull yet higher would required that you sport a velvet trench coat and a pimp hat (with obligatory ostritch feather in said hat) with the aforementioned chin-fro and you would obtain crazy-mad pull.
